Other than your premium, everything you pay out of your pocket for medical care counts toward your out-of-pocket limit (or maximum). This includes copayments, coinsurance, and deductibles. The explanation of benefits (EOB) document from your insurance company will give running total of what you’ve paid. Once you hit the out-of-pocket limit, the insurance company will pay 100% of your medical expenses for the rest of the year.
